Understanding the Causes of Persistent Nasal Congestion

Nasal congestion is a common symptom that many people initially associate with colds, allergies, or seasonal irritation. However, when congestion keeps returning or persists despite standard treatments, there may be an underlying condition contributing to ongoing symptoms.

What is Nasal Congestion?

Nasal congestion occurs when the tissues lining the nasal passages become swollen and inflamed, narrowing airflow through the nose.

While mucus can contribute to symptoms, congestion is often related to inflammation and swelling within the nasal tissues and blood vessels. When swelling persists for an extended period, it may begin interfering with breathing comfort, sleep quality, and daily activities.

Common Causes of Nasal Congestion

Short-term congestion is often related to temporary illness or irritation. Persistent congestion may be associated with underlying conditions such as:

Allergies

Environmental allergens such as pollen, dust mites, pet dander, or mold may trigger inflammation and mucus production inside the nose.

Chronic Sinusitis

Long-term inflammation of the sinuses may contribute to impaired drainage and persistent congestion.

Deviated Septum

A septum that shifts significantly to one side may chronically restrict airflow through the nasal passages.

Nasal Polyps

Soft, noncancerous growths inside the nasal passages or sinuses may obstruct airflow and drainage.

Enlarged Turbinates

Turbinates are structures inside the nose that help filter and humidify air. Persistent swelling of the turbinates may reduce airflow.

Non-Allergic Rhinitis

Some patients experience nasal inflammation triggered by smoke, weather changes, strong odors, hormonal fluctuations, or environmental irritants rather than allergies.

Environmental Irritants

Ongoing exposure to smoke, chemicals, pollutants, or dry air may contribute to chronic nasal irritation and congestion.

In many patients, more than one contributing factor may be present at the same time.

Symptoms That May Accompany Nasal Congestion

Depending on the underlying cause, congestion may occur alongside symptoms such as:

  • Difficulty breathing through one or both sides of the nose
  • Postnasal drip
  • Facial pressure or fullness
  • Reduced sense of smell or taste
  • Frequent throat clearing or mild cough
  • Sleep disruption or snoring
  • Dry mouth from mouth breathing during sleep
  • Headaches that worsen when bending forward

Nasal obstruction during sleep may contribute to snoring and, in some cases, obstructive sleep apnea.

When Nasal Congestion May Require Evaluation

Temporary congestion during a cold or seasonal illness is common. Persistent symptoms may warrant further evaluation when congestion:

  • Lasts longer than 10 days without improvement
  • Returns repeatedly throughout the year
  • Consistently affects one side of the nose
  • Does not improve with over-the-counter therapies
  • Occurs with facial pain, recurrent sinus infections, or reduced sense of smell

Congestion lasting 12 weeks or longer may be considered chronic and can be associated with structural or inflammatory conditions that require additional evaluation and treatment approaches.

Patients should also be aware that overuse of certain nasal decongestant sprays may lead to rebound congestion, a condition called rhinitis medicamentosa.

How the Cause of Congestion Is Evaluated

Evaluation begins with a review of your symptoms, medical history, environmental exposures, and possible allergy triggers.

A physical examination of the nasal passages may help identify inflammation, structural narrowing, or other abnormalities. In some cases, nasal endoscopy — a quick in-office procedure using a small camera — may be recommended to evaluate the nasal passages and sinus openings more closely.

Imaging studies may also be used when structural concerns or chronic sinus disease are suspected.

Because patients with similar symptoms may have very different underlying causes, treatment recommendations are most effective when tailored to the specific source of congestion.

Treatment Options for Nasal Congestion

Treatment recommendations depend on the underlying cause of congestion and each patient’s clinical findings.

Allergy-Related Congestion

Treatment may include antihistamines, nasal steroid sprays, saline rinses, or allergy immunotherapy for appropriate candidates.

Structural Nasal Obstruction

When a deviated septum or enlarged turbinates significantly restrict airflow, minimally invasive or surgical treatment options may be discussed.

Chronic Sinusitis

For select patients with chronic sinusitis, balloon sinuplasty may be considered to help improve sinus drainage.

Nasal Polyps

Anti-inflammatory medications are often considered first. Procedural or surgical options may be discussed when polyps significantly obstruct airflow or sinus drainage.
